It is good practice to have an enrolment form for the children who join your group. This enables you to identify emergency contact numbers and any information that is relevant to ensure that the child can participate safely in the activities you provide.

It is important that this information is stored in a secure location and that any confidential information provided e.g. medical needs are only shared with the staff or volunteers who need to know this information to ensure the safety of the child.
