CPD Request Form (CPD/R)

This should be completed by staff seeking approval by Head Teachers to take part in a staff development activity. Head teachers should supply budget codes where appropriate. These should only be completed for staff development activities. Once completed these should be sent to the CPD office at Hayfield House.

Group Staff Development Activity Form (GSD)

Generally, these are completed by promoted members of staff who are organising a development activity for a group of staff members. Once completed these should be sent to the CPD office at Hayfield House.

Employee Travel Request form

These should be completed by all staff members travelling off Shetland on Council related business. Once completed these should be returned to the CPD office at Hayfield House.

Direct Flight Booking Form

This form should be used by and member of staff requireing an inter-island flight. This should be filled in and passed to the CPD office 7 days before travel.

Claim for Reimbursement of Expenses

This should be completed on return to Shetland and should be relevant to a single trip south. These should be completed and returned to Hayfield House by the fifth of each appropriate calendar month. Full information on completion is printed on the reverse side of the form. Expense claims must be submitted on original forms only. Photocopied forms will be returned.

Off Shetland Feedback

This should be completed and returned with the expenses claim form. This informs the education service of the value of events and supports formation of a record of expertise.

Course Evaluation

This is completed at the end of a Shetland based training event. This informs the Education Service of the merits of a particular course and the benefits of its repetition.

Course Impact Evaluation

This will be sent to members of staff approximately six months following a training event in Shetland. The purpose of this is to establish the impact of the training on the professional practice.

Certificate of Attendance

These are issued to participants at training events within Shetland. These are not intended to be certificates of merit or proof that any particular standard has been achieved. They are intended to be retained in a member of staff’s CPD portfolio as a record of training undertaken.
