Request for Involvement
Following a consultation with someone from the Education Outreach Group where agreement for a request for involvement was reached then the Request for Involvement form should be completed.
For school age children this is usually completed by the relevant staff member in school (head teacher/ASN teacher). For Pre-School children and those requiring support for vision or hearing it is completed by a health professional. Parents can also submit a request but only following consultation with someone from the Education Outreach Team.
The relevant Education Outreach Group (ASN) professional will then follow up on the request by liaising with the person / service requesting involvement. Existing Local Authority and service processes will then be followed to ensure that parents are fully involved in the process.
